Our Lady of Perpetual Help Church

Registration to become a member of Our Lady of Perpetual Help is as easy as filling out a card.  You can drop by the Parish Center and fill one out, or print out the form below, fill it out and drop it by after Mass.  Should you have further questions, please call our main number at 480-947-4331.
