2013-2014 Registration Requirements
The following forms and fee must be returned to our office in order to be considered for acceptance into Our Lady of Perpetual Help Catholic School. Click on the document to access and print a copy.
- Registration Form
- Gesell Screening Results (Grades K-1), if taken at another school
- Letter of Recommendation (Grades 2-8)
- Request for Student Records
- Private School Affidavit of Intent (notarized)
- Diocese Emergency Form (one per child)
- OLPH Emergency Form ( one per child)
- $100 Non-Refundable Registration Fee (per student)
- Copies of the following documents must also be provided:
- Certified Birth Certificate
- Current Immunization Records
- Baptismal Certificate
- First Reconciliation, First Eucharist, Confirmation Certificates (if applicable)
- Last Year’s and Current Year’s Report Cards
- Most Recent Standardized Test Scores
- Any Court-Ordered Custody/Visitation Directive
When all registration materials have been received, you will be notified within one week of your child’s acceptance into Our Lady of Perpetual Help Catholic School. We reserve the right to administer placements tests and the right of grade placement should the need arise.
